Draw Volume: 10 mL
Minimum Volume: 5 mL
Collection Instructions: Do NOT draw blood unless in possession of a completed test requisition including consents.

Draw blood in an EDTA (purple-top) tube(s) for the patient and both parents. Send at least 5cc (children) or 10cc (adults). Parent test code is PARTRI.
Collection Cut-Off Time: Samples must be received by TCH Pathology by 1400, Monday - Friday to ensure same day shipping.
Special Collection Instructions: Parent's blood needs to be collected along with the child's and sent together whenever possible. Parental blood testing is performed at no charge but parent should be registered and the test code PARTRI ordered.
While it is preferred that both parents and child are collected and submitted together, this is not required. When submitted individually, a completed Baylor test requisition including consents should be present for collection and transmitted with specimen to the reference section for processing.

WHOLE EXOME SEQUENCING REQUISITION from BCM should be filled out completely relating to clinical patient information and consent for testing. CONSENT SECTION OF REQUISITION (pages 4-8) MUST BE SIGNED BEFORE SPECIMEN WILL BE FORWARDED TO THE BAYLOR LAB FOR TESTING.
